8 February 1989 - Indep. Air 1851

Flight IDN 1851 departed Bergamo at 10.04 UTC for a flight to Punt Cana (Dominican Republic) via Santa Maria (Azores). At 13.5:47 Santa Maria Tower cleared the flight to descend to 3000ft for a runway 19 ILS approach: "Independent Air one eight five one roger reclear to three thousand feet on QNH one zero two seven and runway will be one niner." After a brief pause the message resumed at 13.56:59: "expect ILS approach runway one niner report reaching three thousand." At exactly the same time the flight crew read back to Santa Maria: "reclear to two thousand feet and ah... (pause) ... one zero two seven." Because of the overlap in communications, the tower controller didn't notice the crew's mistake and the crew didn't receive the final part of the request to report at 3000ft. The aircraft continued the descent and at 14.08 the GPWS sounded for 7 seconds, without any comment or reaction from the crew. The aircraft then crashed into the Pico Alto mountain at an altitude of 1795ft.
